Trachoma

It is a chronic keratoconjunctivitis, affecting the superficial epithelium of conjunctiva and cornea simultaneously.

         

Causes-

1. Chlamydia trachomatis.

2. Infants are affected more.

3. It is more common in areas with dry and dusty weather.

4. More common in unhygienic conditions.

5. More common in area abundant in flies.

6. Unsanitary conditions.

7. Dust.

8. Exposure to smoke, irritants.

9. Main source of infection is conjunctival discharge of affected person.

10. It spread by direct spread, common through flies, though contaminated fingers of nurses, common use of towels, handkerchief.


Symptoms-

1. Insidious onset.

2. Acute conjunctivitis.

3. There is sensation of something sand like in the eyes.

4. Lacrimation.

5. Scanty mucoid discharge.

6. Conjunctival follicles on upper tarsal conjunctiva.

7. Papillary hyperplasia.

8. Conjunctival scarring.

9. Superficial keratitis.

10. Corneal ulcers.

11. Corneal opacity in upper part.


Investigations-

1. Conjunctival cytology.

2. ELISA test.

3. Polymerase chain reaction.

4. Direct monoclonal fluorescent antibody microscopy of conjunctival smear.


Management-

1. Antibiotics.

2. Maintain hygiene.

3. Environmental sanitation.

4. Medicines.

5. Surgery.
