Scleritis

Scleritits is a chronic inflammation of sclera. There is visual impairment and loss of eye if not treated properly or at time. It usually occurs more commonly in females and affects elderly patients.

Causes -

1. Autoimmune collagen disorders.

2. Gout.

3. Thyrotoxicosis.

4. Herpes zoster ophthalmicus.

5. Staphylococcal infection.

6. Streptococcal infection.

7. Tuberculosis.

8. Syphilis.

9. Surgical injury.

Types-

1. Non-necrotizing anterior diffuse scleritis.

2. Non-necrotizing anterior nodular scleritis.

3. Anterior necrotizing scleritis with inflammation.

4. Anterior necrotizing scleritis without inflammation.

5. Posterior scleritis.


1. Non-necrotizing anterior diffuse scleritis
- is very common. It involves a small area of anterior sclera.

2. Non-necrotizing anterior nodular scleritis
- is found near limbus in the form of purplish nodules.

3. Anterior necrotizing scleritis with inflammation
- In it sclera becomes transparent with anterior uveitis.

4. Anterior necrotizing scleritis without inflammation
- It is usually found in elderly females. There is history of rheumatoid arthritis.

5. Posterior scleritis
- occurs in sclera behind the equator.


Symptoms-

1. Severe pain in eyes which radiates to jaw and temple.

2. There is redness in eyes.

3. Watering from eyes.

4. Light is intolerable by eyes.

5. Impaired vision.


Investigations-

1. TLC.

2. DLC.

3. ESR test.

4. Urine examination.

5. Tests to diagnose rheumatoid factor, syphilis, uric acid level.


Treatment-

1. Steroid therapy under doctor's supervision.

2. Medicinal therapy.
