Agranulocytosis

Agranulocytosis is a condition in which the neutrophil count of body gets decreased below 100 per mm3.

         

Agranulocytosis is a condition in which the neutrophil count of body gets decreased below 100 per mm3.

Causes-

1. Decreased neutrophil production in case of bone marrow infiltration or bone marrow injury. Injury to bone marrow can occur due to harmful or side effects of drugs, infections, aplastic anaemia or radiation.

2. Increased neutrophil destruction due to splenomegaly severe sepsis, side effects of drugs and Felty’s syndrome.

3. Vitamin B12 deficiency.

4. Folic acid deficiency.

5. Antineutrophil antibodies.

6. Systemic lupus erythematosus.


Symptoms-

1. Necrotic ulcers in throat and mouth are there along with decreased neutrophil count.

2. Sore throat with fever is also there.

3. Toxaemia and septicaemia is there when there is decreased neutrophil count in the body.

4. Bleeding from various organs or bleeding disposition can occur if there is associated thrombocytopenia along with agranulocytosis.

5. Cyclic neutropenia.


Investigations-

1. TLC, DLC test.

2. Peripheral blood smear test.

3. Bone marrow test.

4. Culture test of various body fluids.

5. X-ray and ultrasound of affected organs to localize the site of infection.


Treatment-

1. Remove the aggravating factor.

2. Find and treat the cause of disease.

3. Broad spectrum antibiotics.

4. Chemotherapy.

5. Recombinant granulocyte colony stimulating factor and granulocyte monocyte colony stimulating factor are administered in high risk patients under doctor’s supervision but avoid using it twenty four hours before or after administering cytotoxic chemotherapy.
